Hours for all offices:
Monday thru Thursday: 8:00 am to 4:30 pm
Friday: 8:00 am to 12:00 pm (noon)

You may contact us by phone or e-mail at any time during or after business hours. We review after hours voice mail and e-mail each morning and answer the phone throughout the day.

If it is an emergency you should go directly to the closest emergency room. If you are already a patient at Consultants in Pain Medicine, inform those in attendance at the emergency room.

Making an Appointment
Your initial appointment with Consultants in Pain Medicine is just the first step in understanding you, your pain and how to proceed with your treatment.

We will work in partnership with your referring physician to develop a plan best suited for you.

It is important to know as much about you and your pain as possible. If you have had any previous diagnostic testing or treatment, seeing these records will be helpful. You can bring these with you to your initial consultation, drop them by our office or fax them to (912) 466-0366.

During your initial consultation we will ask you to fill out brief questionnaires to help us understand the nature of your pain. If you wish to complete the forms prior to your first appointment you can download the Patient History Form and the Patient Information Form. We will discuss your responses to the questionnaires and you will have a comprehensive medical evaluation. Please allow a minimum of two hours for your first visit.

Consultants in Pain Medicine makes every effort to see patients as quickly as possible. Our office staff will be glad to assist you in making an appointment. Please call (912) 466-9111 Monday through Friday, except holidays, between 9 a.m. and 5:30 p.m.

What to Bring to an Office Appointment
The following list will help you remember what items and information to bring with you to Consultants In Pain Medicine.

  • New Patient Paperwork mailed to your home. (You will have additional paperwork to fill out once you have checked in. If you need assistance, please plan accordingly and bring someone with you to help complete paperwork).
  • Insurance cards and claims-filing information.
  • Picture ID
  • Any X-Ray, MRI, CT Films or disk (It is patient’s responsibility to locate these items and bring to appointment).
  • Current medications and dosages (Please bring the medication bottles, not a list).
  • Before your visit to CIPM, review your health plan’s booklet or call your insurance company to clarify your policy benefits and requirements. Your policy may vary from another subscriber insured by the same company.

What to Expect at your Office Appointment
Consultants In Pain Medicine will accept and may review written medical records, radiology films or other materials from other health-care providers if related to your medical care at CIPM. If you wish to retain your non-CIPM records for your personal files, please bring a duplicate set to your appointment. All radiology films will be returned to you unless the physician requests they be stored in our files.

Length of Your Visit
No two patient visits are exactly alike. The length of your stay will depend on the number of tests ordered and the complexity of your medical conditions. It’s our goal to schedule your appointments so that your evaluation won’t be unnecessarily delayed. In some cases a return visit may be necessary.

Friends and Relatives
Friends and relatives are welcome to accompany you to CIPM. They will be asked to remain in the waiting areas, unless there are special reasons for them to stay with you during the examinations and tests.

Billing and Insurance Information
This information is provided to help you understand the financial and insurance aspects of your visit to Consultants In Pain Medicine. Please note that relationships with insurance companies change routinely, and patients should contact their plan’s representative to determine coverage and if CIPM is in or out of network with their plan. Payment for all services provided is due at the time services are rendered. However, as a courtesy, CIPM will submit a claim for you to any insurance carrier. Any fees not payable by the insurance carrier are the patient’s responsibility.
 

Pre-certification
Certain services may require pre-approval from your health insurance company before they can be performed. Before your visit to CIPM, review your health plan’s booklet or call your insurance company to clarify your policy benefits and requirements. Your policy may vary from another subscriber insured by the same company. It is important for you to be aware of your plan’s pre-certification requirements to receive the full benefits allotted for services performed. Our pre-certification staff can help you obtain authorization/approval for services performed during your evaluation at CIPM. However, pre-certification is your responsibility.

Plans that require Pre-certification
If your insurance plan requires pre-certification for testing or procedures, it is your responsibility to advise registration staff of those requirements. If pre-certification is required, Pre-Certification staff will call your insurance company or primary care physician (PCP) for approval. Pre-certification is not done unless you notify our Registration staff to do so. If pre-certification was required but not done, your benefits may be reduced or denied and become your responsibility in full.
 

There are many commercial or traditional health insurance plans with which CIPM does not participate. If your insurance plan does not participate with CIPM, it is difficult to predict what will be paid. You must check with your employer, health plan or other appropriate party about benefits before coming to CIPM. It is important that you know your health plan benefits.

Workers’ Compensation
Consultants in Pain Medicine is committed to seeing injured workers within 48 hours of injury. Our primary goal is to provide the highest quality of care, returning the worker to the work force in a timely manner. Our physicians have occupational medicine training and understand the Workers’ Compensation System.

A complete assessment is done on each patient and, if necessary, referrals to other specialists are made. A treatment plan is formulated and communicated to the assigned case worker and/or employer. We believe it is paramount that case managers and adjusters have the information they need and know the caliber of the physicians with whom they do business. We at Consultants in Pain Medicine strive to provide what the patient needs and move forward in resolving each case.
